Science Fair Forms and Information
Copies of the science fair forms can be downloaded below.
All students must submit forms 1A and 1B. Parent/guardian signature are needed.
Form C is for students who are conducting an experiment that involves human test subjects. Everyone who is participating in the experiment must sign this form. If the student is under 18, a parent or guardian also needs to sign the form.
Form D is for student projects that need to have adult supervision. The adult supervisor needs to sign this form

Background Research
You must conduct background research into your topic so you can learn what is already known about the topic. The research will vary quite a bit depending on what you want to focus on. For each source that you find, you must write down the bibliographical information. Try to get a variety of sources including journals, magazines, books, websites, interviews, etc. All information must be recorded in your journal!
